  1. Stop the infusion.

  2. Decrease the infusion by 6mL.

  3. Do an aPTT or PTT 6h.

  4. Notify the doctor.

  1. It takes 3-5 days to reach the therapeutic concentration of Coumadin.

1. Talk to the patient's family about the patient's right to decide (Rationale: As a nurse make sure that the patient has all the information he needs to make an informed decision. Advocating for the patient's right is a key nursing role, especially when the family disagree with his wishes. Then you should support his decision).

1. Troponin level (best serum indicator of MI) "Gold standard test".

CASE V

Question 1:

  1. Take pulse before each dose of digoxin.

  2. Do not take the dose if pulse is less than 60 bpm, notify MD.

  3. Take it same time of the day.

  4. Take missed dose only up to 12 hours later.

  5. Notify MD for S/S of digitalis toxicity (fatigue, weakness etc…)

  6. Do not take it with antacids, laxative and cold/cough medications.
